Why did so many companies jettison healthcare coverage for their employees during The Bush Years?

It became cost prohibitive. Prior to the wartime wage freeze during WWII it was rare to have a company offer healthcare coverage. Henry J Kaiser was one of the industrialists who had an HMO within his company. Employer provided healthcare started off as an incentive, when did it become a requirement?
